Managing a chronic illness like heart failure, asthma, or diabetes can be difficult. At Sullivan County Community Hospital, we care about helping you manage your chronic illness successfully. We recognize the best health outcomes require coordinated interventions that go beyond a hospital or doctor’s visit.
Care Coordination Nurses
Care Coordination in the primary care practice involves deliberately organizing patient care activities and sharing information among all of the participants concerned with a patient’s care to achieve safer and more effective care outcomes.
What CC Nurses do:
- Provide routine calls or visits to see how you are doing
- Provide medication education
- Help improve your safety
- Help arrange services to help you in your home
- Help you make appointments
- Give you information on resources available in your community
- Help you if you have been in the hospital
- Help schedule preventive screenings
- Work with you to establish health goals
- Provide education on your health conditions such as fall prevention, exercise, and nutrition
Certified Community Health Workers
CCHWs deal directly with social determinants of health, the non-medical issues that influence health outcomes. You do not need a provider referral or insurance to work with our CCHWs.
What CCHWs do:
- Helps obtain food and/or clothing
- Conducts home visits to make sure home is safe and secure
- Educates and supports patients after hospital stays
- Provides help paying utility or medical bills
- Connects with social services to help patients
- and so much more.
Perinatal Nurse Navigators
Stress negatively impacts your heart. Speaking with someone who can help you identify and eliminate (or reduce) sources of everyday stress can be beneficial.
What PNNs do:
- Develops a plan for pregnancy, delivery, and bringing baby home
- Provides support for all types of pregnancies
- Provides nutrition education and childcare principles
- Provides breastfeeding instruction and support
- Supports mother and baby until baby is 15 months old
- Connects mother to community resources, if needed.

Do you have to see a provider on SCCH campus to be enrolled in the Chronic Care Management (CCM) Program?
You do have to see a provider on our campus for CCM; however, you do not need to see a provider on our campus to receive services from our Certified Community Health Workers or Perinatal Navigation Services.
Can I get my annual Medicare Wellness visit done at Care Coordination?
Yes. We are the only department on campus who performs the annual Medicare Wellness visit.
